Nuclear Cooperation in Latin America

Representatives from 19 Latin American and Caribbean countries met today in Vienna during the 54th IAEA General Conference to celebrate the 25th anniversary of ARCAL, a cooperation agreement for the promotion of nuclear science and technology in the region.

ARCAL provides a framework for Member States collaboration with the support of the IAEA and other international co-operation entities. It addresses key development priorities focusing on pressing needs related to food security, human health, environment, energy and industry, and radiological safety.

"We are celebrating 25 years of ARCAL and the progress achieved in all these years," said Ambassador Carlos Barros, Permanent Representative of Uruguay to the IAEA.

"From now on we need to expand the agreement´s framework and maximize its reach. We have eighteen projects scheduled for the biennium 2012-13 and we are prioritizing them so that every country and the region in general can profit fully from nuclear technologies."

The work of ARCAL has been crucial and innovative because many countries lack adequate know-how in the field of science and technology.

"Many of our countries lack the technology to treat cancer, for example, and nuclear technology has been vital to our efforts to eradicate this ailment which has such a great impact in the region," says Liliana Solís Díaz, Director General of Costa Rica´s Atomic Energy Commission.

"Other applications include the use of isotopic techniques for identifying hydrological and geothermal resources and determining the extent of their contamination. This has enabled Costa Rica to establish policies to protect these valuable resources."

"In the area of agriculture, my country has been able to increase quality and yield of such a vital crop as rice."

The IAEA has supported ARCAL since its inception in 1984.

José Antonio Lozada, Programme Management Officer for the IAEA Division of Latin America, Department of Technical Cooperation, spoke of ARCAL as one of the best examples of South-South cooperation among developing countries.

"Countries with a more advanced level of nuclear knowledge host fellows from neighbouring countries to train them," he said.

Areas of cooperation span across human health, food safety, environment, energy and radiation safety.

"ARCAL focuses on radiotherapy, nuclear medicine and the treatment of infect contagious diseases, as in the prompt detection of ailments such as dengue," explains the IAEA?s Lozada.

"By accelerating detection, patients can be treated more rapidly and effectively."

"In the area of nuclear energy, ARCAL has enabled the technological transference in the use of energy planning models so that each country can predict what its energy demand would be and find ways to meet its needs."

ARCAL technical cooperation projects provide capacity-building through expert missions, training courses, meetings, scholarships, scientific visits and workshops.

ARCAL provides a framework for Member States in Latin America and the Caribbean to intensify their collaboration through programmes and projects focused on the specific shared needs of its members.

It was established in 1984, and was made a formal inter-governmental agreement in 1998.

Between 1984 and 2009, over 1 000 professionals and technicians have been trained through a total of 72 ARCAL projects in industry, radiochemistry, radiation medicine, nutrition and soil and water management.

The acronym stems from its Spanish name (Acuerdo Regional de Cooperación para la Promoción de la Ciencia y la Tecnología Nucleares en América Latina y el Caribe).

Cancer Epidemic Calls for New Agenda Setting

Can we hope that significantly more resources and much greater awareness can be mobilized to make cancer control in the developing world a global health priority?

Cancer experts gathered at the Scientific Forum during the 54th IAEA General Conference to answer just this question. Although the situation looks grim - cancer has not received the forceful attention this ravaging disease warrants, nor has it been included among the Millennium Development Goals - the panellists and experts in the audience agreed that priorities can be changed. With sufficient political resolve, cancer can become a priority issue on the global health agenda.

Franco Cavalli, Chairman of the Scientific Committee of the European School of Oncology, noted that many governments are afraid to talk about cancer because to be effective, cancer care and control requires a health care system that is robust from the ground up. And many countries simply do not have such a system.

Cavalli also said efforts by non-governmental organisations, civil society and international organisations will never be successful, unless national governments are interested and enthusiastic about curing cancer, which many view as a challenge that is too expensive or too complex to overcome.

Eduardo Cazap, President of the International Union for Cancer Control, said it is necessary to have a wider perspective when combating cancer, since the lack of access to treatment plagues the poor and uninsured in all countries, even rich, developed nations.

Background

According to the International Agency for Research on Cancer, in high income countries, cancers of the colorectum, lung, breast, prostate and stomach dominate. In low- and middle income countries, cancer of the lung, stomach, breast, liver, colorectum and cervix are dominant.

However, this pattern is changing rapidly in many parts of the world, where lung and breast cancers, once historically uncommon, are increasing considerably.

The continued growth and ageing of the world´s population, in combination with exposure to cancer risks such as tobacco use, will greatly exacerbate the future cancer burden. By 2030, it is expected that there will be over 21 million incident cases of cancer, 13 million cancer deaths annually, and 75 million persons alive with cancer.

If cancer is not given higher priority through focused global efforts, health-care systems in less developed countries will face even more burdensome challenges as the number of cancer cases increase.

Worldwide, about four billion X-ray exams, 35 million nuclear medicine exams and eight million radiation therapy treatment courses are undertaken each year.

With millions exposed to ionizing radiation for medical purposes, and developing countries acquiring more machines to treat and diagnose cancer, the safety of patients is an ever-increasing concern.

During the IAEA´s Scientific Forum, cancer experts and regulators put their heads together to explore the problems and possible solutions relating to the safe and appropriate use of new radiation medicine technology in developed and developing countries.

Pierre Scalliet, Chairman of the Department of Radiation Oncology at the Cancer Center of the St. Luc University Hospital in Belgium, said the vast majority of accidents are caused by lack of training and safety culture, not by faulty equipment or lack of the right equipment.

Agnés Buzyn, a nuclear regulator who is Chairperson of the Board of Directors of Institut de Radioprotection et de Sûreté Nucléaire in France, agreed that staff training and a cuture where safety is valued are at least as important as effective regulation and safety standards.

She proposed that professional societies, manufacturers, patients and governments should all be involved in radiation safety: from staff training to improving patient awareness.

Panellists and experts from the audience then went on to discuss ways to ensure that doctors, physicists and the medical facilities they work for adhere to safety regulations. Although it was generally agreed that the differences in society and culture mean that one size will never fit all countries, experts concluded that the best results could be achieved if compliance with patient safety regulations was linked to doctors´ pay and/or professional accreditation.
